New Patient Forms

If you’re a first-timer, you won’t have to fill out our dental forms while you wait for your appointment. Instead, we will email or text your forms to be filled out ahead of time after setting up your initial appointment. You can do it from home at your convenience. 

We have included a few forms below that may be necessary prior to your appointment if you have had treatment elsewhere or you would like to designate a guardian to accompany a minor child to their appointment. These forms are printable, can be submitted electronically, or can be faxed to us at 503-400-6236.
